[T10] SAS T10/1562-D revision 04 has two version descriptors

As part of building a modern database for INQUIRY Data Version 
Descriptors, the presence of two Version Descriptors has been noted for 
SAS T10/1562-D revision 04, specifically 0BFAh and 0BFBh.

Since keeping the sanity check that caught this seems good for future 
Version Descriptor assignments and since Version Descriptors are almost 
never made obsolete, the following "adjustment" is being made with what 
ought to be no normative change in SPC-6.

SAS T10/1562-D revision 4 --> 0BFAh
SAS T10/1562-D revision 04 -> 0BFBh

Expect to see this change (if you look for it hard enough) in SPC-6 r08.
