[T10] SPL-4 - Simplified text change resolution for public letter ballot comment 16-253r1

I've uploaded a proposal for  a simplified way to fix the issue where the BREAK response is technically breaking the rule in the standard, and allows for legacy operation. It also keeps the state transition to BREAK for the PS_request states, which makes for consistency in the way that all of the state machines respond to a BREAK.

The simplified text change requires three changes of "shall" to "should" for SPL-4 ,and no change to figures.
