New revision of state machine rewrite posted

In preparation for the ZBC call this afternoon, a new revision of 113 has
been uploaded, see:

All of the changes requested last week have been incorporated, plus offline
requests from various active players.
An effort was made to ensure that resolved ZBC Letter Ballot comments were
reflected in the new revision, but time ran out of that work and progress
stalled at the end of 4.4.
