SPC-4: Mode page subpage 00 and non-subpage page problem

The FCP WG in reviewing letter ballot comments has discovered some 
ambiguity in SPC-4 related to mode pages.
In Table 359  Summary of mode page codes several legacy mode pages that 
have no subpage are listed as being subpage 00h.  However, when looking at 
those pages they do not include the subpage field and the SPF bit is set 
to zero.  This seems inconsistent and is unclear if it is legal to return 
a page with SPF bit set to one and the subpage field set to zero.
Either the referenced table is in error or there is a concept not 
described sufficiently.
