SBC-2 revision 6 now available

SBC-2 revision 6 incorporates the approved proposal from last CAP/T10 
meetings and updates its command tables to include new (relative to SBC-1) 
SPC-2/3 commands, SCC commands, and correct the SMC-2 commands.

Proposals expected in July include:
* remove RESERVE/RELEASE since they're now obsolete in SPC-3
* mandate PERSISTENT RESERVE IN/OUT   (SSC-2 is doing so;
   replaces the previously mandatory RESERVE/RELEASE)
* mandate REPORT LUNS  (SSC-2 is doing so)
