Recommended CDB structure

When looking in the latest revision of SAM, I noted that there no longer is a
set of tables that illustrate the recommended CDB structure for 6, 10, 12, and
16 byte CDBs. Has this been moved somewhere else (SPC perhaps) or has this
information simply been deleted?
 
I believe this information has to be described in a SCSI-3 document. If it
doesn't exist any more, I strongly request that the information be
re-introduced.
 
My problem that brought me to this - is there a recommended structure for 16
byte CDBs? I am proposing a command that needs 16 bytes and I'd like to make
sure that I am following the SCSI-3 guidelines.
