For (1), the first TEST UNIT READY with terminate as you describe. However
the second TEST UNIT READY may have GOOD status (as you describe) or it may
have CHECK CONDITION status with NOT READY sense data. The point here is
that there are other valid reasons for following TEST UNIT READY commands
to terminate with CHECK CONDITION status.
For (2), you should expect the same response after many resets with no
intervening commands that you would get after one reset. SCSI standards
allow for queuing multiple unit attention conditions and reporting each one
sequentially, but a reset should clear the queue back to one reset unit
attention.
